Support from compassionate behavioral health professionals is critical to overcoming barriers and staying in control of daily choices. Those living with a mental health disorder need community based services that focus on providing the full spectrum of assistance required to be contributing members of their communities.

Mental Health/Substance Use Recovery Services

Maintaining control over daily life and choices often takes consistent, repeated effort. Venture Forthe understands that living with a mental health diagnosis can impact the life of an individual and the lives of their family. Our mental health and recovery services are ideal for individuals who have struggled with other treatment and rehabilitation options, and require specialized, compassionate, home and community based care, and support.

Once submitted, one of our Outreach Coordinators will respond within a 24 hour period to confirm client eligibility and make contact with the client within 72 hours to begin our intake process. Home and Community Based Services (HCBS)

Peer Services

This service connects you to peer specialists who have lived experience of recovery. Your peer will help you:

Have someone who listens to you and understands what you are going through.
Adovocate for yourself and make decisions about your own recovery.
Connect to people and resources in your community.

Life Skills Services

Life skills training services offer the support needed for those with a behavioral health diagnosis to live as independently and productively as possible.

Who is Eligible for CORE?

HCBS eligibility is determined through a state eligibility assessment. Some qualifying criteria include:

Have a Managed Medicaid Plan
21 years of age or older
Have a qualifying behavioral health diagnosis (serious mental health and/or substance use diagnosis)
Prequalifying coding designated by NYS Medicaid
