Managed Long Term Care

Medicaid Managed Long Term Care (MLTC) is a program designed to delivery long term services for those who are chronically ill or disabled, but want to stay in their homes.

MLTC plans have a comprehensive range of covered services for those who will require care for at least 120 days (from date of enrollment). Venture Forthe provides long-term home health care services based on eligibility through your long-term care plan.

At Venture Forthe, MLTC may cover long-term home health care for seniors or those with a chronic illness or disability, and Health Home services.

Who is Eligible for MLTC?

Enrollment in an MLTC plan requires:

  • A chronic illness or disability that qualifies you for nursing home care*
  • At least 120 days of care requirements
  • At least 65 years old (PACE organization only – 55+ years)
    • Be 18-21 years old and dual-eligible for Medicaid and Medicare

  • Enrollment in Medicaid (some plans accept Medicare)
  • The ability to safely stay at home at time of enrollment

Nursing home level of care is typically defined as requiring assistance with 3 Activities of Daily Living (ADLs) or cueing for 2 ADLs if they have dementia or Alzheimer’s Disease.  ADLs include:

  • Bathing
  • Grooming/Personal hygiene
  • Dressing
  • Toileting

  • Eating (not including cooking)
  • Transferring (functional mobility – walking, getting in and out of bed or a chair, etc.)
